Pandora Gilboa Local Schools
New student enrollment process:
Parents wishing to enroll a new student into the Pandora-Gilboa School District need to use the following process:
1. Fill out the New Enrollment Data Sheet and the Emergency Medical Authorization Form , Authorization for Prescribed Medication- Physician Statement, Authorization for Non-Prescribed Medication.
2. Review the Immunization Summary for School Attendance.
3. Gather the following records:
a. Birth Certificate
b. Vaccination Record
c. Custody Agreement (if applicable)
4. Visit our building office (bring with you the forms and documents listed above at 410 Rocket Ridge, Pandora, Ohio 45877 during our regular business hours (8 a.m. to 2:30 p.m. in June and August or 7:30 a.m. to 3:30 p.m. during the school year. Our building offices are closed to the public in July. Our friendly office staff will assist you through the remainder of the process.
